p. 175The Lock Hospital, which adjoins the Work-house, was removed from Grosvenor-place to its present site, in 1842. This institution was founded in 1737, and no less than 60,502 patients have been treated at this Hospital since that date. The number of in-patients for 1851, was 388; of these 193 were females, and 195 males; during the same period 785 persons were attended to, as out-patients. ”

“ Lock Hospital (The) , which formerly stood on the site of Grosvenor Place Houses, was built in 1746, and patients admitted on January 31st, 1747, for the first time. The Institution included an asylum for the reception of penitent females, founded in 1787, and a chapel, built in 1764, with the primary view of aiding the income by its pew rents. The chapel was always celebrated for the powerful and popular preachers who occupied its pulpit, among whom may be mentioned Martin Madan, Thomas Scott, editor and commentator of the Scriptures, and C. E. De Coetlogon ”

“ At the north end of Westbourne Park Road are national schools.
St. Paul's Church and schools stand in Marlborough Street. The church was built in 1873, and is of earth-brick, without spire or tower. This part of Paddington is considerably cut up both by the railway and canal. Crossing the latter at the Lock Bridge, we see the Lock Hospital and Asylum standing on the west side of the road. The hospital was established in 1737, and the asylum in 1787. Adjoining the hospital is the workhouse, occupying with its infirmary about [Pg 105] 5 acres. The workhouse has 623 beds, and the infirmary 280. ”

“ There are two houses belonging to this hospital, the one in Kent Street, called the Lock, and the other at Kingsland, whither such unfortunate people as are afflicted with the French disease are sent and taken care of, that they may not prove offensive to the rest; for surely more miserable objects never were beheld, many of them having their noses and great part of their faces eaten off, and become so noisome frequently, that their stench cannot be borne, their very bones rotting while they remain alive. ”

“ If we close the ward to visitors ... we have never in all the years the hospital has been in existence done ... that! Public confidence is our greatest asset. Has been. What shall we do? The newspapers, the police, the reputation of the hospital, d’y’see?”
“Too well, sir.”
But the tension was wearing itself out in speech and Dr. MacArthur went on:
“The hysteria among the nursing and medical staffs was bad enough, God knows, but before today is over, we must face the hysteria manifesting itself among the menial staff. How can a hospital run without orderlies, electricians, cooks? ”

“ Thomas' Hospitals, now in process of construction at the Surrey Side of the Thames, in Lambeth, opposite the Houses of Parliament, will combine a number of hospitals for Special Diseases, and will accommodate about 2,000 patients, with as many beds, and will have an income of £50,000 a year, or more.
It is impossible to think of any disease, complaint, deformity, or injury to any member or organ of the body, which has not its special hospital or institution for relief or cure, in the English metropolis. ”

“ My Richmond friends never lost sight of me for one day during my service in that city. Nearly every day my little boy was sent for to play among happy children, far away from the impure atmosphere of the hospital, which was soon filled with patients suffering from almost every form of disease.
As the demand for more room became pressing, the three stories of the main building were successively utilized, as well as a large storage-room in the yard. The ground-floor contained the surgeons' and steward's offices, store-rooms, etc., while the second and third formed two immense sick-wards. ”

“ Our plan, we reasoned, was analogous to the custom of “private” hospitals, which give free treatment or charge according to the resources of the ward patients. Both private hospitals and visiting nursing are thereby lifted out of “charity” as comprehended by the people.
We felt that for economic reasons valuable and expensive hospital space should be saved for those for whom the hospital treatment is necessary; and an obvious social consideration was that many people, particularly women, cannot leave their homes without imperiling, or sometimes destroying, the home itself. ”

“ Often, too, these diseases rose to the height of epidemic pestilences, so that patients, however extreme their need, dreaded the very name of hospital, and the most skilful surgeons distrusted their own craft. New hospitals or new wards were built, yet after a very short time the new became as pestiferous as the old; and even scrupulous care in ventilation and housemaids’ cleanliness failed to prevent the devastation. Surgery had enlarged its freedom, but only to find the weight of its new responsibilities more than it could bear. ”

“ The existing system of asylum structure, management, and treatment makes this almost unattainable. No class of cases requires the attention of trained nurses more than subjects of recent insanity."
Can the present State hospitals provide such accommodations and give such care as Dr. Tuke claims the acute insane for their proper treatment need? I believe they can, and also that the accommodation and care there provided could be made far better than any that might be furnished in an institution established exclusively on the lines of a general hospital. ”

“ In overcoming this prejudice, a great deal depends upon the character of the institution itself. It is not enough to establish hospitals:—they must be attractive and comfortable to such a degree that they become highly desirable to prospective patients. They must be well run, well managed, the food must be good, and the patients well treated. To obtain segregation, we must have hospitals which offer great advantages over the home.
Home Care of the Advanced Case. If there are no hospital facilities, it then becomes the nurse’s duty to give nursing care to the bed-ridden patient. ”

“ Boyce was for setting out for France forthwith. I dissuaded her from a project so embarrassing to the hospital authorities and so fatiguing to herself. In spite of the chivalry and humanity of our medical staff, old ladies of seventy are not welcome at a busy base hospital. As soon as he was fit to be moved, I assured her, he would be sent home, before she could even obtain her permits and passes and passport and make other general arrangements for her journey. There was nothing for it but her Englishwoman's courage. ”

“ When the woman is confined in her own home, the nurse is engaged to remain at least two weeks after the baby is born, and three or four weeks, if the family purse will permit.
In cities the custom of going to a hospital for confinement is growing steadily. It is recognized that the hospital is cheaper, safer and more convenient. The sanitation is perfect, danger from infection is practically nil, and there are expert nurses and house physicians at hand should an emergency arise. It is cheaper because the hospital equipment is at the command of all patients ”

“ Such disadvantages might be entirely removed if parliament would decide to throw open every poor-law infirmary for clinical purposes, and to have connected with each such establishment a responsible visiting medical staff, consisting of the best qualified men to be found in the community which each hospital serves. The old prejudice against hospital treatment has disappeared, for the least intelligent members of the population now understand that, when a citizen is sick, there is no place so good as the wards of a well-administered hospital. ”

“ Another problem is the question of creating, in connection with General hospitals, wards where patients of this sort may receive at the beginning of their treatment, their preliminary examination, where they may be evacuated home with compensation or without compensation, or evacuated to a prolonged treatment hospital for further care.
It has sometimes been difficult to get enough men, trained personnel, to man these wards. ”

“ The disadvantages, inseparable from general hospitals, are such as can never be counterpoised by all the regularities of medicine and regimen. Nature and kind nursing save a much greater proportion in our plain way, at a smaller expense, and with less abuse. One branch only of hospital institution is wanting with us; that is, a general establishment for those laboring under difficult cases of chirurgery. ”

“ During its two years of work for the sick and wounded, no military hospital has succeeded in establishing a finer record. To see it is a wonderful experience. The hospital consists of 17 wards, with 578 beds, and is entirely staffed by women—surgeons, doctors, pathologists, oculists, dental surgeons, anæsthetists, dispensers, nurses, orderlies. The only men are the patients.
Sir Alfred Keogh, the Director-General of the Army Medical Service, said, when speaking of it: “The hospital is in every respect a military hospital, differing in no way from any other military hospital in the country. ”

“ New hospitals are erected under the eaves of old hospitals. Sick and injured people are carried long, unnecessary distances at great risk. Seven new hospitals are going [Pg 632] up in Pittsburgh and yet, when they are all completed and other changes which have been decided upon are carried out, there will be a great belt of river wards, thickly populated, without a convenient hospital plant,—wards in which we shall see disease is most rife. ”

“ The company erected at a cost of more than $60,000 a thoroughly modern brick hospital of thirty-five bed capacity which furnishes absolutely free to its employees and their dependents every type of medical, surgical and obstetric 78 treatment. No charge is made for any hospital care, operating fees, or for medicines. ”

“ The glorious halls and corridors, the massive marble pillars, the splendid marble staircases, indicate wealth, but the perfection of the sanitary and scientific arrangements tells of long years of anxious thought and study and research. So perfectly is this marvellous hospital built that, in the event of plague or cholera breaking out to such an extent as to infect the building, the whole of the inside of the hospital can be removed and another hospital will still be left standing as complete as the other. ”

“ The Pennsylvania Hospital was also a model, for it contained a department for the insane, the only one of the sort in America except the Virginia Lunatic Asylum at Williamsburg. Even there the treatment of these beings, whom a later instinct of humanity thought peculiarly worthy of care and lavish expenditure, was harsh enough,—strait-jackets, whip­pings, chains, and dark-rooms being a part of the pre­scribed treatment in every such hospital in the world but where no hospitals existed, as in New England, New York, and elsewhere, the treatment was apt to be far worse. ”

“ At least two physicians should reside in the building. It is unfortunate that reception buildings as a rule are entirely too small. They should be large enough so that the acute and recoverable cases, as well as those found on observation not to require hospital treatment, can be returned to their homes without any further contact with the hospital or the necessity of a protracted residence with the chronic and purely custodial cases. ”

“ Apart from its interest as a new feature which all new hospitals should adopt, the gatehouse or admission block has an importance in the wider sense, that it may come to form the key to the solution of the problem of how best to provide hospital accommodation for the poor in great cities under the best hygienic conditions, while protecting them from the misery and danger of prolonged delay in first treatment, especially in connexion with accidents and other cases of urgency. ”

“ The character of cases treated and the nature of operations performed embrace all the variety of diseases to which women are peculiarly liable, and for the treatment of which this institution was established.
The general health of the hospital has been remarkably good, not a single death occurring among the patients admitted during the year. ”

“ The C.C.S., usually in huts or tents, is the first real hospital behind the firing zone. It may have accommodation for a couple of hundred patients; is supplied with X-Ray equipment, a well-arranged operating room with expert surgical assistance, and is the nearest place to the line that trained nurses are sent. Here for the first time since he left the line the patient gets all those little motherly attentions that only a woman can give. The injured man may be kept here days, weeks, or even months if he happens to be a case that would be endangered by moving. ”

“ The current system of large hospitals is abandoned, as being equivalent to 'warehousing diseases on the largest possible scale;' while special hospitals are deemed unnecessary—'as if the different organs could walk out of the body and present themselves for separate treatment.' Each hospital has an ambulance ready to be sent out to bring any injured persons to the institution; the ambulance drives straight into the hospital, where a bed of the same height on silent wheels receives the patient, and conveys him or her to a ward. ”
