“ Of the latter, those known as “saprophytes” are regarded as specially indicating the presence of sewage contamination.
Certain varieties of bacteria have been found associated with some forms of disease, and particular attention has been bestowed upon the study of these germs. The biological examination of water consists of pathological experiments on living animals, made by injecting a solution of the water-residue beneath the skins of rabbits, etc., and of experiments made by inoculating culture gelatine with the water. ”

“ The dissemination of biological agents also poses difficult technical challenges. Whereas persistent chemical agents such as sulfur mustard and VX nerve gas are readily absorbed through the intact skin, no bacteria and viruses can enter the body via that route unless the skin has already been broken. Biological agents must either be ingested or inhaled to cause infection. To expose large numbers of people through the gastrointestinal tract, possible means of delivery are contamination of food and drinking water, yet neither of these scenarios would be easy to accomplish. ”

“ Patient decontamination team members first apply the 5 percent hypochlorite solution to the patient's clothing. Then remove the patient's clothing as in decontamination of chemical agent patients. Bandages, tourniquets, and splints are not removed. Place patient's clothing in a plastic bag and dispose in the contaminated waste dump.
d. Decontaminate the patient's skin. Have the patient bathe with soap and warm water or apply the 0.5 percent hypochlorite solution. If the patient is unable to bathe himself, a member of the [G-20] decontamination team must bathe him. ”

“ The air is at certain times and places contaminated by the products of respiration and the bodily emanations of diseased persons.
In certain diseases, commonly known as contagious, organic matters are thrown off by the lungs and skin of the sick, which tend to reproduce these diseases in the bodies of other persons. The exact nature of these poisons is in most cases unknown, but they are generally believed to be living microscopic organisms (bacteria, bacilli, micrococci, etc.) , which multiply their kind in the blood of the person who has inhaled them. ”

“ The Causes therefore of these Diseases must either be generated in the Air, or produced from the Effluvia of Animal, Vegetable, or Mineral Substances floating in it. And consequently the Effects of the Contagious Particles must be extreamly various, according to the Qualities of the Bodies from which they are produced.
When any of these Causes is of so deleterious a Nature, as not only to be Infectious, but to destroy all or most of those that are affected by it, That Disease is called a Pestilence. ”

“ The small quantity of diseased organic matter originally introduced into the system by absorption, acts as a ferment and reproduces itself in the mass of the blood, until this becomes unfitted for the performance of its functions and the animal is killed; the active principle being thus copiously present, is exuded from the skin and lungs and gives a contagious character to the disease, or it remains only in the blood, or is secreted in pustules, constituting infection, by which the disease may be communicated to some other person. ”

“ Shall we content ourselves with finding a heap of decomposing vegetable or animal matter, or rest satisfied by discovering an escape of sewer-gas from some untrapped drain into one or more of the houses? Since we—the reader and I—are scientific and reasonable people, we can not adopt the theory that the disease is contagious—an opinion held by some able but rather easy-going physicians—and having once effected a lodgment in the community is capable of infecting all who have systems favorable to the development of the poison. ”

“ We have the action of the bacteria themselves, and also of their products or toxins. In particular diseases, now one and now the other property comes to the front. In bacterial diseases affecting or being transmitted mostly by the blood, it is the toxins which act chiefly. The convenient term infection is applied to those conditions in which there has been a multiplication of living organisms after they have entered the body, the word intoxication indicating a condition of poisoning brought about by their products. ”

“ One destroys the germs and the other moves them through the blood and carries them off with the waste products of the body.
The white corpuscles thus stand as the defenders of the body, ready to destroy the germs as they enter, and are, for each individual, the best of all preventives of germ diseases. The person whose blood is lacking in white cells is always liable to "catch" contagious or infectious diseases, and the one who has that element of the blood in proper proportion is best fitted to withstand disease. ”

“ They gain an entrance through the lungs, food canal, or skin, and, living upon the fluids and tissues, multiply with great rapidity until they permeate the entire body. Not only do they destroy the protoplasm, but they form waste products, called toxins, which act as poisons. Diseases caused by germs are known as infectious, or contagious, diseases.129 The list is a long one and includes smallpox, measles, diphtheria, scarlet fever, typhoid fever, tuberculosis, la grippe, malaria, yellow fever, and others of common occurrence. ”

“ First we may notice food as a distributor. Food may become contaminated by infectious material in many ways; for example, by contact with sewage, or with polluted water, or even with eating utensils which have been used by patients. Water is also likely to be contaminated with infectious material, and is a fertile source for distributing typhoid and cholera. Milk may become contaminated in a variety of ways, and be a source of distributing the bacteria which produce typhoid fever, tuberculosis, diphtheria, scarlet fever, and a few other less common diseases. ”

“ In just what manner the germ of consumption gains entrance to the human body, we are more or less uncertain, but there are reasons for the belief that in many instances they pass in by means of the inhaled air; there is no doubt that in a small percentage of cases the bacillus gains entrance to the body through an abrasion of the skin or of some mucous membrane; finally the bacteria are [Pg 182] often taken in with the foods that we eat, or by putting objects upon which the germs are present into the mouth, or eating with hands which have been contaminated and not washed. ”

“ But the poisonous action of most of the pathogenic bacteria is due to the chemical products secreted by the organisms, and it is to the circulation and absorption within the body of these poisons that the disturbances of the animal system, which characterise disease, decay, and dissolution of every organism, must be traced. ”

“ Methods of fighting Germ Diseases.—As we have seen, diseases produced by bacteria may be caused by the bacteria being directly transferred from one person to another, or the disease may obtain a foothold in the body from food, water, or by taking them into the blood through a cut or a wound or a body opening.
It is evident that as individuals we may each do something to prevent the spread of germ diseases, especially in our homes. We may keep our bodies, especially our hands and faces, clean. ”

“ Cellular pathology with its wonderful revelation of the anatomical seat of disease and bacteriology, with its still more wonderful revelation of the external cause of disease, so dazzled the eye and the mind that we forgot that the sensitive animal body behind the attacking microbe had its changes, too, its changes in body chemistry that could not be stated in terms of cells and bacteria. The pendulum is swinging back now to a consideration of the constitution of the body on which the microbe or poison acts, its resistance or immunity, its anaphylaxis or allergie. ”

“ But, be that as it may, the micro-organisms themselves, by their own bodily presence, set up changes and perform functions which may have far-reaching effects. It is obvious that the wider the distribution the wider is the area of tissue change, and vice versâ. Yet there is something of far greater importance than the mere presence of bacteria in human or animal tissues; for the secondary action of disease-producing germs—and possibly it is present in all bacteria29—is due to their poisonous products, or toxins, as they have been termed. ”

“ We know, now, that many diseases are spread by means of germs which are carried from person to person by various means, such as air, water, milk, and other food; discharges from the mouth, nose, bowels, bladder, wounds; clothing; the hands; the breath, and so forth.
It has been found that great heat, intense cold, sunshine and some powerful drugs called disinfectants kill germs. Germs thrive and multiply in dirt, dampness and darkness. That is why it is important to have fresh air, sunshine and cleanliness in order to keep well, and to help in curing those who get sick. ”

“ His media are sterilized by the most elaborate precautions; everything in which the germs of disease delight, not merely morbid matter, dirt, decay, but even healthy atoms of animal or vegetable substance, which, having been alive, are subject to decomposition, are to be excluded, saving just such a simple minimum as may serve to keep the microbe alive and multiplying. ”

“ Those who hold to the doctrine of contagion admit that, to give such contagion efficacy in the production of wide spread results, filth or decaying organic matter is essential; while those who sustain the theory of non-contagion—the production of the poison from sources without the bodies of the sick—contend that it has its entire origin in such filth—in decomposing matter, especially in fermenting sewage, and decaying human excreta. ”

“ There is a class of intoxication diseases which depend on toxins generated by germs whose habitat is the soil, water, or other external media, and whose germs do not enter the human body as a necessary feature in their life-histories, although their toxins may. ”

“ It is a thoroughly established fact that the disease is caused by a special type of bacteria and that if the bacteria could be killed outside the body, no transmission of the disease could occur. It is also true that if the disease germs could be destroyed within the body the patient would recover immediately, provided the toxins had not been already distributed through the system. ”

“ Every particle of animal or vegetable matter, even if only a single grain in weight, by exposure to the air, putrefies, breeds, and attracts to itself thousands of microbes, and becomes a center of infection. Thus, in a piece of street dirt containing organic matter, we may find upon examination, the germs of typhoid fever, diphtheria, scarlet fever, or consumption. ”

“ So far as one can at present see ahead, the next advances must be in the direction either of means which shall fortify the human organism against the inroads of bacteria, or disease-germs, or else in the discovery of substances, such as we do not yet know, which shall be at the same time poisonous to the germs and innocuous to the patient, to whom they may be administered in doses sufficient for their purpose. ”

“ This organic matter may consist not only of the products of animal and vegetable life but also of certain microscopic forms of living organisms which are likely to accompany such products. Contagious diseases are known to be due to the presence in the body of minute living organisms or germs. Each disease is caused by its own particular kind of germ. ”

“ We have now reached a phase of this question when it is of the very last importance that light should once for all be thrown upon the manner in which contagious and infectious diseases take root and spread. To this end the action of various ferments upon the organs and tissues of the living body must be studied; the habitat of each special organism concerned in the production of each specific disease must be determined, and the mode by which its germs are spread abroad as sources of further infection. ”

“ These consist of minute organisms that find their way into the body and, by living upon the tissues and fluids and by depositing toxins (poisons) in them, cause different forms of disease. Besides destroying germs that may be present in the blood, the white corpuscles also leave the blood and attack germs that have invaded the cells. By forming a kind of wall around any foreign substance, such as a splinter, that has penetrated the skin, they are able to prevent the spread of germs through the body. ”

“ We know that the agents responsible for infectious diseases are specific minute organisms which must be present in the system to produce the disease in question, and that these micro-organisms are conveyed from place to place by infected persons, by intermediaries, on articles to which they have attached themselves, in contaminated food, in drinking-water, and in many other ways. ”

“ In order to destroy any germs which may have entered into the cut from the instrument, it is well to wash out the wound with some mild disinfectant, such as very dilute carbolic acid or hydrogen peroxide, and then to bind the wound with a clean cloth, to prevent later entrance of germs.
234. Chemicals as Food Preservatives. The spoiling of meats and soups, and the souring of milk and preserves, are due to germs which, like those producing disease, can be destroyed by heat and by chemicals. ”

“ COLDS, CONSUMPTION, AND PNEUMONIA
Disease Germs. In all foul air there are scores of different kinds of germs—many of them comparatively harmless, like the yeasts, the moulds, the germs that sour milk, and the bacteria that cause dead plants and animals to decay. But among them there are a dozen or more kinds which have gained the power of living in, and attacking, the human body. In so doing, they usually produce disease, and hence are known as disease germs. ”

“ By some scientists, digestion is regarded as a species of fermentation, probably due to the action of lifeless ferments; e.g. sucrose cannot be taken into the system, but is first fermented to glucose.
334. Most Infectious Diseases are now thought to be due to parasites of various kinds, such as bacteria, microbes, etc., with which the victim often swarms, and which feed on his tissues, multiplying with enormous rapidity. Such diseases are small-pox, intermittent and yellow fevers, etc. Consumption, or tuberculosis, is believed to be caused by a microbe which destroys the lungs. ”
