Because we meet with such atypical instances, we are not therefore justified in inferring that it is by a mere arbitrary sport of nature that in the woman a great mammary development is normally associated with the development of the ovaries, and that in man the growth of the beard is associated with the development of the testicles. ”

“ This broadening of the penis results in the female's external genitals being subjected to much more pulling and pushing during the performance of pelvic thrusts. With each inward thrust of the penis, the clitoral region is pulled downwards and then with each withdrawal, it moves up again. Add to this the rhythmic pressure being exerted on the clitoris region by the pubic region of the frontally copulating male, and you have a repeated massaging of the clitoris that — were she a male — would virtually be masturbatory. ”

“ The hymen may be occasionally imperforate and require incision. Cysts and painful carunculae may occur on the clitoris. Any part of the vulva may be the seat of new growths, simple or malignant.
Diseases of the Vagina.— (1) Malformations. The vagina may be absent in whole or in part or may present a septum. Stenosis of the vagina may be a barrier to menstruation. (2) Displacements of the vagina; (a) cystocele, which is a hernia of the bladder into the vagina; (b) rectocele, a hernia of the rectum into the vagina. The cause of these conditions is relaxation of the tissues due to parturition. ”

“ A well-grown and abundant supply of fine pubic hair is a possession highly prized by women, of which they are justly proud, though few of them would acknowledge the fact, even to themselves. None the less it is a fact.
The female sex organs, speaking generally also, are as follows: The vulva, or outward portion of the parts; the vaginal passage; the uterus, or womb, and the ovaries. All but the first named lie within the body of the woman. The vulva is made up of several parts which will be named and described later. The vaginal passage is a tube, or canal leading from the vulva to the womb. ”

“ Owing to the fact that both male and female organs develop from more primitive structures which were sexually undifferentiated, a fundamental analogy in the sexual organs of the sexes always remains; the developed organs of one sex exist as rudiments in the other sex; the testicles correspond to the ovaries; the female clitoris is the homologue of the male penis; the scrotum of one sex is the labia majora in the other sex, and so throughout, although it is not always possible at present to be quite certain in regard to these homologics. ”

“ There may be simply a union of the perfect body of one sex with the susceptibility to such sexual charms as ordinarily attract the other sex alone, or with the mental traits of the other sex. Or the individual may possess the male genitals, but be beardless, or else possess mammary glands, broad pelvis, and sacral dimples; or possessing the female genitals, have a rudimentary moustache, or else meagrely developed breasts, narrow pelvis, etc. Instances have been known of human beings with an ovary on one side of the body and a testicle on the other, and of males who were able to suckle infants. ”

“ The female organs may be most conveniently studied by tracing them inwards from the cloaca. The vagina (v.) opens immediately posterior to the vas deferens, and like it is lined by a continuation of the external cuticle. After passing about half-way across the segment it bends backwards and terminates in a small cyst, the receptaculum seminis (fig. 1, E, r.s.) ; this receives and stores up the male fertilizing elements, retaining them until the ova are ripe. From its posterior extremity there passes a thin-walled canal, wider than the vagina (f.) ”

“ There being four varieties of it, according to Leonides; three of them occur in men and one in women. In men, sometimes about the perineum and sometimes about the middle of the scrotum, there is the appearance of a female pudendum with hair; and in addition to these there is a third variety, in which the discharge of urine takes place at the scrotum as from a female pudendum. In women there is often found above the pudendum and in the situation of the pubes the appearance of a man’s privy parts, there being three bodies projecting there, one like a penis, and two like testicles. ”

“ The conical penis finds its way in the reflected fold of the vagina, while the point of the uterus may be two or three inches in some other direction, making impregnation wholly impossible; besides, in the normal-shaped penis, the corona acting as a valve, behind which the circular muscular fibres of the vagina close themselves, tends to retain the seminal fluid in front, while the very shape of the organ assists in straightening out the vaginal canal and to bring the uterus in proper position. ”

“ Shorn of its natural protective covering, the muco-cutaneous covering of the glans soon becomes true dermoid tissue. Mr. J. Hutchinson (loc. cit.) speaks of 'every one who is acquainted with the effects of circumcision in rendering the delicate [Pg 39] mucous membrane of the glans hard and skin-like.' Contact with the clothing, &c., necessarily induces a chronic inflammatory condition of the part, followed commonly by contraction and condensation. We cannot with impunity rashly interfere with any of Nature's dispositions, however seemingly insignificant. ”

“ In handling or other manipulation of the genitals, without actual intercourse. If the genital organs of a female have merely been gently handled, without any more violent manipulations, we shall not, as a rule, either in the case of children or of adults, find any local changes as a result; contrariwise, if the handling has been rough and brutal, if the fingers have been forcibly thrust within the vulval cleft, or if the pudendum has been pulled and rent, we may expect to find excoriations, redness, swelling, laceration of the hymen, or even of the vagina and the perineum. ”

“ With the assistance of an enlarged clitoris, with which one woman performs the sexual act by introducing the organ within the vagina of another. In France in the nineteenth century a woman is said to have lived whose genital organs were so formed that, on the one hand, as a woman she played the passive part in intercourse with men, and, on the other hand, was able to give sexual gratification to women by assuming the active part of the male. ”

“ What actually happens in an act of union should be known. After the preliminaries of love-play, the stimulated penis, erect, enlarged and stiffened, is pressed into the woman's vagina. Ordinarily when a woman is not stimulated, the walls of this canal, as well as the exterior lips of soft tissue surrounding its exit, are dry and rather crinkled, and the vaginal opening is smaller than the man's extended organ. ”

“ Intromission of the penis may be rendered quite impossible by excessive size of the labia majora, consequent upon elephantiasis, in which disease there is enormous hypertrophy of the subcutaneous connective tissue. New growths may have the same result, fibroids, for instance, lipomata, and cysts, which may attain a remarkable size in the cellular tissue of the labia, the mons veneris, and the perineum, and also in the nymphæ and in the cellular tissue between the clitoris and the urethral orifice. ”

“ If the husband is devoid of previous experience in sexual matters, maladroit attempts at intercourse are exceedingly likely to occur. The penis is thrust in the wrong direction, pressing against either the anterior or the posterior commissure of the vulva. Very often, moreover, the position of the vulva, which is subject to very striking individual variations, is concerned in the production of vaginismus. There are many women in whom the vulva lies in part in front of the symphysis pubis, so that the lower border of the symphysis lies below the urethral orifice. ”

“ The fluid is generally glairy, sometimes thick and gelatinous, often turbid and dark coloured. Not unfrequently the main cyst is subdivided, either by membranous septa, or by an aggregation of smaller cysts of the nature of hydatids. The swelling is at first on one side, and gradually rises out of the pelvis; often it remains long moveable; it increases, becomes more fixed, and ultimately fills the abdomen, displacing the viscera, and giving rise to feelings of much uneasiness, deformity, and loss of health. ”

“ She has a man’s head in every particular of physiognomy and expression, having in the latter figure a full beard and the peculiar intellectual development of a male sage; she has the hairy breast of the man, with the mammary development of the female, and an abnormally-enlarged clitoris, which was often mistaken for the male organ. ”

“ Even in cases in which the loss of substance of the breasts is apparently small, the glandular tissue of the organs has really disappeared, and has been replaced by fat. In advanced age, the breasts become quite small, wrinkled, flaccid, and dependent, and sometimes atrophied to become mere cutaneous folds. The nipples project more prominently, they are darker in colour, and their surface is wrinkled. In the genital organs, the fat disappears from the mons veneris, which becomes flattened, whilst the pubic hair ceases to be curly, and much or all of it is ultimately shed. ”

“ Some operators avoid this bleeding by opening the cyst with the cautery-knife. In any case the bleeding is usually slight if a thin portion of the cyst-wall is selected for the incision. If the ovary is filled with a number of cystic cavities, it is safest to remove the whole organ. If the woman be young and anxious for children, the portion of the ovary that contains the cysts may be excised and the wound in the ovary closed by sutures of fine catgut. Simple puncture of the cysts does no good. The conservative operation is especially desirable in case both ovaries are diseased. ”

“ A rudimentary condition of both ovaries, or bilateral atrophy of these organs, with or without associated atrophy of the entire reproductive system, commonly entails sterility. In such cases, in addition to amenorrhœa, we usually find that the breasts are but slightly developed, the pubic hair is scanty, the labia majora and labia minora are small, whilst sexual appetite is deficient, and during coitus the woman is entirely passive. ”
