“ Establishment of a Provider Network.— Health care providers participating under Medicare are participating providers in the public health insurance option unless they opt out in a process established by the Secretary. ”

“ Disciplining and inspecting entrepreneurial and private forms of medicine and articulating them with the national health service, in such a way as to ensure adequate standards of efficiency and quality in both public and private healthcare institutions ”

“ Promote care coordination between providers of services and suppliers that transition health care providers away from fee-for-service based reimbursement and toward salary-based payment. ` (v) Supporting care coordination for chronically-ill applicable individuals at high risk of hospitalization through a health information technology-enabled provider network that includes care coordinators, a chronic disease registry, and home tele-health technology. ”

“ The nature of protections needed for individuals under a system of bundled payments to ensure that individuals receive quality care, are furnished the level and amount of services needed as determined by an appropriate assessment instrument, are offered choice of provider, and the extent to which transitional care services would improve quality of care for individuals and the functioning of a bundled post-acute system. ”

“ Definition- In this section, the term `health care entity' includes an individual physician or other health care professional, a hospital, a provider-sponsored organization, a health maintenance organization, a health insurance plan, or any other kind of health care facility, organization, or plan. (c) Construction and Treatment of Certain Services- Nothing in subsection (a) shall be construed to apply to, or to affect, any limitation relating to-- (1) the withholding or withdrawing of medical treatment or medical care; (2) the withholding or withdrawing of nutrition or hydration; (3) abortion ”

“ One of the cornerstones of a strong healthcare system is the provision of competent and efficient medical professionals. We will exert all efforts to improve the welfare of our doctors, our nurses, and other medical frontliners. ”

“ Private health care payors. `` (3) Responsibilities.—In accordance with subsection (b) (5) , the clinical prevention stakeholders board shall— `` (A) recommend clinical preventive services for review by the Task Force; `` (B) suggest scientific evidence for consideration by the Task Force related to reviews undertaken by the Task Force; `` (C) provide feedback regarding draft recommendations by the Task Force; and `` (D) assist with efforts regarding dissemination of recommendations by the Director of the Agency for Healthcare Research and Quality. ”

“ And then our efforts to reform health care, to provide health care to all Americans: For the first time ever, a committee of Congress has recommended private health insurance for every American family. ”

“ PRIMARY CARE PROVIDER- The term `primary care provider' means a clinician who provides integrated, accessible health care services and who is accountable for addressing a large majority of personal health care needs, including providing preventive and health promotion services for men, women, and children of all ages, developing a sustained partnership with patients, and practicing in the context of family and community, as recognized by a State licensing or regulatory authority, unless otherwise specified in this section. ”

“ The term `hospital-based' means, with respect to an eligible professional, a professional (such as a pathologist, anesthesiologist, or emergency physician) who furnishes substantially all of the individual's professional services in a hospital setting (whether inpatient or outpatient) and through the use of the facilities and equipment, including qualified electronic health records, of the hospital. ”

“ HEALTH BENEFITS- The term `health benefits' means medical, surgical, hospital, prescription drug, and such other benefits as shall be determined by the Secretary, whether self-funded, or delivered through the purchase of insurance or otherwise. ”

“ Establishing community-based health teams to support small-practice medical homes by assisting the primary care practitioner in chronic care management, including patient self-management, activities. ` (ix) Assisting applicable individuals in making informed health care choices by paying providers of services and suppliers for using patient decision-support tools, including tools that meet the standards developed and identified under section 936 (c) (2) (A) of the Public Health Service Act, that improve applicable individual and caregiver understanding of medical treatment options. ”

“ In General- The Secretary, acting through the Patient Safety Research Center established in section 933 (referred to in this section as the `Center') , shall establish a program to provide grants or contracts to eligible entities to implement medication management (referred to in this section as `MTM') services provided by licensed pharmacists, as a collaborative, multidisciplinary, inter-professional approach to the treatment of chronic diseases for targeted individuals, to improve the quality of care and reduce overall cost in the treatment of such diseases. ”

“ No senior should have to hesitate to get the preventive care they need. That's why my plan will eliminate the deductible in all copayments for all preventive services, for cancer, diabetes, osteoporosis, and other diseases. ”

“ Managed care has transformed medicine in America — driving down costs, but threatening to drive down quality as well. I think we ought to say to every American: You should have the right to know all your medical options — not just the cheapest. If you need a specialist, you should have a right to see one. You should have the right to emergency care if you're in an accident. ”

“ Services You will also be entitled to all forms of treatment in general or special hospitals, whether as an in-patient or as an out-patient. These include, for instance, maternity care, sanatorium treatment, care of mental health, and all surgical operations. The help of consultants and specialists of all kinds will be made available to you as national resources allow, whether at hospital, at special health centres, or at home.
Your doctor will arrange this help when you need it. ”

“ ALLIED HEALTH PROFESSIONALS- The individual is employed full-time as an allied health professional-- (A) in a Federal, State, local, or tribal public health agency; or (B) in a setting where patients might require health care services, including acute care facilities, ambulatory care facilities, personal residences and other settings located in health professional shortage areas, medically underserved areas, or medically underserved populations, as recognized by the Secretary of Health and Human Services.' ”

“ The second and most urgent element consists in the organization of hospital and consultative expert services for all, which, while greatly increasing each patient’s prospect of prompt recovery, will enable the general practitioner to escape from the soul-destroying inefficiency of unaided medical practice. ”

“ IN GENERAL.—A group health plan, and a health insurance issuer offering group health insurance coverage, that provides coverage for surgical benefits shall provide coverage for outpatient and inpatient diagnosis and treatment of a minor child’s congenital or developmental deformity, disease, or injury. A minor child shall include any individual who is 21 years of age or younger. ”

“ IMPROVED TREATMENT STANDARDS, CLINICAL GUIDELINES, DIAGNOSTIC PROTOCOLS, AND CARE COORDINATION PRACTICE- Each Center shall collaborate with other Centers in the network to-- (A) develop and implement treatment standards, clinical guidelines, and protocols that emphasize primary prevention, early intervention, treatment for, and recovery from, depressive disorders; (B) foster communication with other providers attending to co-occurring physical health conditions such as cardiovascular, diabetes, cancer, and substance abuse disorders ”

“ In today's health care system, insurance companies call the shots. They pick whom they cover and how they cover them. They can cut off your benefits when you need your coverage the most. They are in charge. ”

“ TREATMENT OF QUALIFIED DIRECT PRIMARY CARE MEDICAL HOME PLANS- The Secretary of Health and Human Services shall permit a qualified health plan to provide coverage through a qualified direct primary care medical home plan that meets criteria established by the Secretary, so long as the qualified health plan meets all requirements that are otherwise applicable and the services covered by the medical home plan are coordinated with the entity offering the qualified health plan. ”

“ Dentist, Physician Assistant, Pharmacist, Behavioral and Mental Health Professional, Public Health Professional, and Nurse Training- The Surgeon General shall establish provisions applicable with respect to dental, physician assistant, pharmacy, behavioral and mental health, public health, and nursing students that are comparable to those for medical students under this section, including service obligations, tuition support, and stipend support. ”

“ The relatively large death-figure in the medical reports is explained by this statement.
Such cases of illness are the more serious that they are all, in fact, in the most urgent need of hospital care. To replace the latter as much and as efficiently as possible, is the task of our physicians. They visit the patients as often as necessary, and prescribe for them, at the expense of the Society, the requisite medicines. In the treatment of such patients our physicians encounter difficulties which it is possible to remove only partially. ”

“ We scarcely realize how far we have gone in the socialization of medicine. It is impossible to go back, or to stand still. The services of the medical profession are needed, not only to provide the necessary service, but in helping to determine its conditions. One essential item will be the substitution for fees during sickness of an annual payment to private practitioners by each family for supervising its members in health, for inquiry into their industrial and domestic life, so far as it contains elements inimical to health, and for giving preventive more than curative advice. ”

“ Patients are guaranteed a quick independent review of their case, and new federal remedies to hold their health plans accountable. They get a strong new set of rights in our health care system without driving up the cost of health insurance and discouraging employers from offering coverage. ”

“ When, again, an out-patient of the better class was entitled, from the severity of his ailment, to receive the advantages of a consultation by the medical staff, no method existed whereby this aid could be rendered to him, and his transfer afterwards to the care of a medical practitioner attached to some provident dispensary, or resident near the patient’s home, could be properly carried out. ”

“ To secure medical aid any hospital fund may independently, or in conjunction with other local funds, organize and maintain its own ambulatories, hospitals, etc., as well as enter into agreements with individual physicians and establishments. ”

“ Dissemination of Information Through Providers- The Secretary, acting through the Centers for Disease Control and Prevention, shall develop and implement a plan for the dissemination of health promotion and disease prevention information consistent with national priorities, to health care providers who participate in Federal programs, including programs administered by the Indian Health Service, the Department of Veterans Affairs, the Department of Defense, and the Health Resources and Services Administration, and Medicare and Medicaid. ”

“ It will provide you with all medical, dental, and nursing care. Everyone—rich or poor, man, woman or child—can use it or any part of it. There are no charges, except for a few special items. There are no insurance qualifications. ”

“ The Secretary shall establish procedures to make beneficiaries and providers aware of the requirement that a beneficiary complete a health risk assessment prior to or at the same time as receiving personalized prevention plan services. ”

“ An advance care planning consultation with respect to an individual may be conducted more frequently than provided under paragraph (1) if there is a significant change in the health condition of the individual, including diagnosis of a chronic, progressive, life-limiting disease, a life-threatening or terminal diagnosis or life-threatening injury, or upon admission to a skilled nursing facility, a long-term care facility (as defined by the Secretary) , or a hospice program. ”
