In either sex it may spread in the course of years to the pubes, over the perineum, and into the rectum, the recto-vaginal septum in the female ultimately breaking down. At times a profuse watery discharge exudes and even drips from the surface of the new growth, soiling the clothes, soddening the skin, and emitting a peculiarly offensive odour. ”

“ These organs and the bladder are then pulled up firmly toward the head of the cadaver, and with the cartilage-knife hugging the pubic bone the rectum is cut just above the internal sphincter, and the urethra just anteriorly to the prostate. What are the Parts appropriated to Generation in Women? ”

“ That generated by the man is called "prostatic flow;" that produced by the woman "pre-coital secretion."
Now, if time is given for this fluid to be secreted and exuded, all the parts become covered or saturated with it, and they are admirably equipped for easy union. The glans penis is then covered with the slippery fluid, and the vulva and all the walls of the vagina are laved with the substance. At the same time, the vaginal walls have widened and grown soft, and all the parts of the vulva (which are yet to be named and described in detail) are in like condition. ”

“ The cavity of the pelvis contains the bladder, the rectum, and some of the generative organs peculiar to each sex and some windings of the small intestine; they are partially covered by the peritoneum (lining membrane of the abdominal cavity) .
Anatomy of the Female Genital Organs.—The external genital organs, to which the term vulva is usually given, consist of the mons veneris, labia majora, labia minora, clitoris, vestibule, meatus urinarius, hymen, fossa navicularis, fourchette and perineum. ”

“ Sexual Utilization of the Anal Opening.—It is even more obvious than in the former case that it is the loathing which stamps as a perversion the use of the anus as a sexual aim. But it should not be interpreted as espousing a cause when I observe that the basis of this loathing—namely, that this part of the body serves for the excretion and comes in contact with the loathsome excrement—is not more plausible than the basis which hysterical girls have for the disgust which they entertain for the male genital because it serves for urination. ”

“ Over the whole body, indeed, it is apt to be more distinctly marked than is usually the case in man. But even if we confine ourselves to the genital region, while in man that portion of the penis which enters the vagina, and especially the glans, is normally the only portion which, even during turgescence, is sensitive to voluptuous contacts, in woman the whole of the region comprised within the larger lips, including even the anus and internally the vagina and the vaginal portion of the womb, [89] become sensitive to voluptuous contacts. ”

“ A long, tight prepuce, eczema of the genitals, accumulations of filth and sebaceous matter around the glans of the penis or of the clitoris, retention of urine beyond the proper time,318 pin-worms in the rectum, hemorrhoids, fissures of the anus, all lead to rubbing, pressing and handling of the genitals.
When the children are old enough to understand, they must be taught never to handle their genitals, the same as they are warned not to poke the fingers into their noses, ears, or eyes. ”

“ With paralysis of the bladder the penis may be pendent out of the sheath, or being retained within it, the urine may dribble constantly into and from that cavity, and the vulva may be soft and flaccid. When the anus is involved, the adjacent part of the rectum usually participates becoming overloaded, the sphincter is soft and lax and allows a constant oozing, and the exposure of the mucosa. The paralytic tail hangs between the thighs, limp and flaccid, and becomes saturated with manure and in females with urine. ”

“ Prostrate Gland.—The prostrate gland is located just before the bladder. It swells in men who have previously overtaxed it, thus preventing all sexual intercourse, and becomes very troublesome to void urine. This is a very common trouble in old age.
6. The Penal Gland.—The penal gland, located at the end of the penis, becomes unduly enlarged by excessive action and has the consistency of India rubber. It is always enlarged by erection. It is this gland at the end that draws the semen forward. It is one of the most essential and wonderful constructed glands of the human body. ”

“ Orifices of the Body. The mouth, nose, ears, anus, urethra and vagina are to be examined with special regard to their condition and contents (open, closed, gaping, torn, bleeding, discharge of pus, blood, mucus, féces, stomach contents, semen, urine, foreign substances, parasites, ear-wax, etc.) . In cases of suspected rape an especial examination of the orifice of the vagina or anus is indicated.
35. Percussion and Palpation. The external examination may be closed by the percussion of the heart, lung, spleen, liver and stomach boundaries, and by the palpation of the abdomen. ”

“ What actually happens in an act of union should be known. After the preliminaries of love-play, the stimulated penis, erect, enlarged and stiffened, is pressed into the woman's vagina. Ordinarily when a woman is not stimulated, the walls of this canal, as well as the exterior lips of soft tissue surrounding its exit, are dry and rather crinkled, and the vaginal opening is smaller than the man's extended organ. ”

“ By this mechanism, the urethra, which usually serves as the canal for the outflow of urine, is made for the time being solely subservient to the purposes of the sexual act. That the outlet from the bladder is obstructed by the swollen caput gallinaginis when the penis is erect, is shown by the familiar fact that a man whose penis is erect cannot pass water, although the way is freely open for the ejaculation of the semen. ”

“ They should always have at hand, and within easy reach, a towel, or napkin, with which to care for the surplus of the seminal emission, which, as soon as the organs are separated, will, in greater or less quantity, flow from the vagina. Some of the same fluid will also remain upon the penis when it is withdrawn. The husband should absorb this surplus which remains with him with the towel, as soon as the organs are parted, and immediately leave his super-imposed position, leaving his wife perfectly free, to do as she will. ”

“ Preparation of the External Genitals and Vagina.—The pubis and the external genitals should be shaved. The woman should be drawn down to the edge of the bed, and the anus, the external genitals, and the vagina should be scrubbed with green soap. The vagina should be washed throughout. The nurse may do this by inserting one or two fingers, or she may retract the perineum with the Sims speculum, and scrub the vagina, the fornices, and the vaginal cervix with cotton held in forceps. ”

“ The female genitals occupy the same relative situation in the pelvis as the male, but they are an antithesis to each other. The male are constructed to deposite, and the female to receive; consequently, in the female there is a conduit or passage, in place of the male penis, termed the vagina, leading to the womb—the receptacle for the impregnating fluid. The vagina is placed between the bladder and rectum. ”

“ More time indeed than the average husband dreams of spending upon it. Yet woman has at the surface a small vestigial organ called the clitoris, which corresponds morphologically to the man's penis, and which, like it, is extremely sensitive to touch-sensations. This little crest, which lies anteriorly between the inner lips round the vagina, erects itself when the woman is really tumescent, and by the stimulation of movement it is intensely roused and transmits this stimulus to every nerve in her body. ”

“ In handling or other manipulation of the genitals, without actual intercourse. If the genital organs of a female have merely been gently handled, without any more violent manipulations, we shall not, as a rule, either in the case of children or of adults, find any local changes as a result; contrariwise, if the handling has been rough and brutal, if the fingers have been forcibly thrust within the vulval cleft, or if the pudendum has been pulled and rent, we may expect to find excoriations, redness, swelling, laceration of the hymen, or even of the vagina and the perineum. ”

“ Of a violent inflammation of the penis, which had originated in unclean intercourse with a beast, with a coruncle, or knot, constricting the urethra, cured in a manner to the last degree empirical and cruel. The penis being laid on a rough stone, the Physician suddenly when the patient was not expecting it, struck it as heavily as ever he could with his doubled fist, so that the stoppage and ulcer might burst. This case has a smack of the Venereal disease about it; and might serve as an argument for those who hold that this disease was known to the Ancients as well. ”

“ The strangury, which has its origin from pain at the neck of the bladder, consists of a pain in the external extremity of the urethra or of the glans penis of men, and probably in the external termination of the urethra or of the clitoris of women ”
