Suction

“ Fig. 31 illustrates a stationary suction cleaning plant applied to cleaning railway carriage cushions, and Fig. 32 shows a similar installation in use in an office building.
Cleaning by Air Blast. By transferring the hose from the suction side to the discharge, a suction cleaner may be used to blow dust from machinery of all kinds and from places that are high up and cannot be cleaned economically by suction. ”

“ Business anxieties and worry also have a very pronounced effect; so that, with the change in the conditions of man and the inception and multiplication of diseased conditions, as well as the creation of constitutional and transmissible diseases, this practice of suction should have been stopped. ”

“ This is not easily explained, and perhaps it is as difficult to understand. An influence upon which others have dwelt, is the suction power of the heart in active dilatation, acting as a vis a fronte (power in front) in drawing blood to it. ”

“ The Australian doctors make great use of the principle of suction, and employ it in all kinds of cases. If, for example, a patient has a bad pain in his stomach from overeating, or suffers more than he thinks right from the blow of a waddy, the doctor sucks at the afflicted part vigorously, and at last produces from his mouth a piece of bone, or some other hard substance, which he asserts to be the concentrated essence of the pain, or other ailment. ”

“ The instrument then will come into direct contact with the child and, with a pressure of approximately 55 or so millimeters of mercury, applied to the end of this instrument, as it is attached to a long, thick suction tubing at this end, and to the abortion instrumentation—that is, the machine—at the other end, the suction tip will begin to tear the child apart. The pieces of the body are torn away, one by one, until finally, all that remains are shards of the body and the head itself. ”

“ When young children continue to suck, or masticate a tasteless body put between their lips, or into their mouths, we may conceive, that the actions of suction and mastication begin to pass from their automatic towards their voluntary state. ”

“ The utmost care to avoid the "sucking in" of any liquid or blood during its birth must be exercised, for this often seriously interferes with the breathing. Sometimes this condition is not relieved until a soft rubber catheter is placed in the throat and the mucus is removed by quick suction. It is said that this aspiratory activity on the part of the uterus may be perceived during coitus by the male also (?) . It is assumed by Grohe that the wave motion of the cilia of the epithelium lining the cervical canal, is of importance in promoting the ascent of the spermatozoa ”

“ Tourniquet—A lightly occlusive tourniquet during a 30- to 60-minute period of incision and suction would seem to possess some advantages. In severe cases where medical attention is hours away, a completely occlusive tourniquet may be necessary to prevent death. Sacrifice of the extremity may be necessary for the preservation of life.
3. Incision and suction—Suction should begin three to five minutes after injection of venom if symptoms of poisoning are present. Incisions one-fourth inch to an inch long across each fang mark should be made in order to open the wound for more efficient suction. ”

“ Mastication thus becomes painful, and even impracticable, in the adult; and suction at the breast or the bottle difficult and painful in the infant. The mouth of the infant is so hot that its heat is imparted to the nipple of the nurse, whose sensations in nursing sometimes furnish the earliest indication of the disease. ”

“ If you place the hand on one of the open ends and exhaust the air, by attaching the long tube of the pump to the other you cannot remove the hand easily. Do not try to pump the air out entirely, as the suction may be too strong and draw blood. It is by the rarefaction of the air that the cupping-glass is applied to people who require bleeding. ”

“ A youthful practitioner, whose last molars have not been a great while cut, meets an experienced and noted physician in consultation. This is the case. A slender, lymphatic young woman is suckling two lusty twins, the intervals of suction being occupied on her part with palpitations, headaches, giddiness, throbbing in the head, and various nervous symptoms, her cheeks meantime getting bloodless, and her strength running away in company with her milk. ”

“ When any one is very sick, and that they be called upon, or perhaps, though rarely ordered in their dreams by their familiars, they sing, blow and suck alternately and with such violence that one would think they wanted to to blow them to the d——l, or swallow them down their throats. But no, it is to force in the medicine of which they have generally a mouthful masticated into a pulp, or something near salve, sometimes. The suction business is to draw out the Devil: the medicine, bone, stone, iron, brass, stick, or whatever they imagine it is, that occasions the disease. ”

“ If commenced in youth, thick lips may be reduced by compression, and thin linear ones are easily modified by suction. This draws the blood to the surfaces, and produces at first a temporary and, later, a permanent inflation. ”

“ Large suction sweepers, worked by a powerful inhaling bellows, which swiftly and silently suck up, from carpet, furniture, and curtains, all particles of accumulated dust, are the perfected instruments chosen ”

“ The accumulation of an excessive amount of inflammatory exudate may endanger the vitality of the tissues by pressing on the blood vessels to such an extent as to cause stasis, and by concentrating the local action of the toxins. Under such conditions the tension should be relieved and the exudate with its contained toxins removed by making an incision into the inflamed tissues, and applying a suction bell. When the exudate has collected in a synovial cavity, such as a joint or bursa, it may be withdrawn by means of a trocar and cannula. ”

“ But the fœtus so soon as it is born, aye, before it is born, will suck; doubtless as it had done in the uterus long before. For I have found by experience that the child delayed in the birth, and before it has cried or breathed, will seize and suck a finger put into its mouth. A new-born infant, indeed, is more expert at sucking than an adult, or than he is himself if he have but lost the habit for a few days. For the infant does not suck by squeezing the nipple with his lips as we should, and by suction in the common acceptation ”

“ A large number of these cases are dependent upon alteration and diseases of the genitalia; but a considerable number are unconnected with local disease, and in many other cases the health is never regained after all local phenomena have disappeared.
Sexual excitement in the woman causes certain congestion of the genital organs; and at the time of the orgasm there is a reflex movement which corresponds to erection, and which consists of a peristaltic movement of the tubes and uterus; to the uterus also is ascribed an act of suction by which the spermatozoa are drawn up into its interior. ”

“ The wound cavity itself is now closed by several rows of buried sutures, so arranged as to bring together the 156edges of the several layers, tissue to like tissue. In congenital cases he does not loosen the sac, but sutures its opposing surfaces down to the point where the testicle lies free. The wound of the skin is now closed by a continuous silk suture, and an iodoform and collodion mixture brushed over the line of suturing until it is perfectly covered in and blood no longer oozes through. No drain is used, and no further dressing is deemed necessary. ”

“ A long straight needle with a spear-point is convenient for introducing the mass-suture. A gauze sponge should be placed beneath the incision as the sutures are introduced, to prevent injury of the intestines and the escape of blood into the peritoneum. When the pad is removed, the omentum, if readily found, should be drawn down behind the incision. Before each suture is secured the sides of the incision should be drawn forward by traction on the ends of the suture, to ensure accurate apposition upon the posterior or peritoneal aspect. ”

“ Sutures, like ligatures, were a common cause of suppuration in or around the edges of the wound. Therefore, in the hope of avoiding this trouble, they were made of silver wire, which was inconvenient to handle, and gave pain at the time of removal of the sutures. At the present time they are of silkworm-gut, catgut, silk or horsehair; they are made aseptic by boiling, and can be left any number of days without causing suppuration and can then be removed without pain. Next may come the consideration of surgical dressings. ”

“ Sutures.—Twisted silk sutures are to be preferred, as they do not invite sepsis, as softening catgut does, and retain the parts during the entire time required for healing, while the latter is liable to become separated by uneven absorption, allowing the wound to gape at that point and causing more or less of a cicatrix, so intolerant to patients of this class.
Dressings.—Bulky bandages are not required; they heat the parts, and look unsightly. The author employs antiseptic adhesive silk plaster for covering all external wounds, except where the hair prevents its use. ”

“ Many young mothers have no prominence to the nipple, so that neglect on the part of the nurse may cause such to lose the benefits of suckling. A friendly adult or child could soon draw out the nipple by sucking so that the babe can get hold; after which the nipple cups should be kept in constant use, till the babe is strong enough to keep the glands soft, and the nipple pliable. Usually, at this juncture, all sorts of teas are suggested: molasses water, milk and sugar and water ”

“ We frequently hear of a babe having no notion of sucking. This “no notion” may generally be traced to bad management, to stuffing him with food, and thus giving him a disinclination to take the nipple at all.
32. How often should a mother suckle her infant?
A mother generally suckles her baby too often, having him almost constantly at the breast. This practice is injurious both to parent and to child. The stomach requires repose as much as any other part of the body; and how can it have it if it be constantly loaded with breast-milk? ”

“ The patient suffers much when the urine is scanty, and contains a large proportion of saline particles. Rest and moderate diet are of paramount importance as means of cure. Support of the penis, by a suspensory bandage, or otherwise, should always be attended to when the patient takes exercise, for many bad consequences will thereby be avoided; indeed it is a measure requisite in all affections of the organ, and particularly in gonorrhœa—when the prepuce, or even the whole penis, is liable to swell enormously. ”

“ If applied early, it is believed they may prevent the formation of pus, as they bring about a resolution of the inflammation. When suppuration has commenced they facilitate the passage of matter to the surface, and lessen the extent of the disease. When applied to an inflamed part or swelling they should extend over considerable surrounding surface, but for a suppurating wound they should be but little larger than the opening. ”

“ More or less pus may appear after the lapse of the first week, and the larger the wound, the more likely this is to happen. And here I would desire earnestly to enforce the necessity of persevering with the antiseptic application in spite of the appearance of suppuration, so long as other symptoms are favorable. The surgeon is extremely apt to suppose that any suppuration is an indication that the antiseptic treatment has failed, and that poulticing or water dressing should be resorted to. But such a course would in many cases sacrifice a limb or a life. ”

“ Shorn of its natural protective covering, the muco-cutaneous covering of the glans soon becomes true dermoid tissue. Mr. J. Hutchinson (loc. cit.) speaks of 'every one who is acquainted with the effects of circumcision in rendering the delicate [Pg 39] mucous membrane of the glans hard and skin-like.' Contact with the clothing, &c., necessarily induces a chronic inflammatory condition of the part, followed commonly by contraction and condensation. We cannot with impunity rashly interfere with any of Nature's dispositions, however seemingly insignificant. ”
