Sores

“ In examining women who have communicated infection, very often no sores are found, and but little unhealthy discharge. In short, the simple elevated sore may arise from the application of secretions from an unbroken surface, from inoculation of matter from a similar sore, or spontaneously, from inattention to cleanliness. Sores with elevated surface, more extensive than those of the glans and prepuce, occur on the skin of the penis and scrotum, or in the folds of the thigh; and in women they are often met with in the perineum, or the cleft of the nates. ”

“ If, indeed, an extensive sore, or a series of sores, be suddenly dried up, the circumstance must be considered as very unfortunate, and the consequences may even be fatal; but such an occurrence is unusual, and the patient may in general be saved by the timely insertion of an issue in the neighbourhood of the sore, or by an active employment of what is best calculated to insure a renewed discharge. Some sores have a disposition to extend by sloughing, and such frequently attack the lips and pudenda of weakly children ”

“ Nothing before her but a living death, the disease steadily growing upon her, until fingers and toes would waste away, her whole body become covered with repulsive sores,—and no power on earth could help her. ”

“ Then the limbs swell, afterward portions of the body, the hands, and the feet; and when the skin can bear no further tension it breaks, and running sores cover the patient, who is repulsive and disgusting to the last degree. ”

“ Bed Sores are the inflamed spots which occur on the body, often as a result of carelessness during a long illness. They are not likely to occur if the bedding is kept smooth and free from wrinkles and the patient kept dry, his position varied as frequently as possible, and the proper bathing not neglected. If such sores threaten there are several remedies which will help to prevent them. Alcohol, brandy or glycerine rubbed over the parts exposed to pressure, after washing in the morning and evening, will serve to harden the place where applied. ”

“ They are found mixed with other organisms in the purulent discharge from the sore, and are chiefly arranged in small groups or in short chains. Soft sores are always contracted by direct contact from another individual, and the incubation period is a short one of from two to five days. ”

“ T. C., born 15th August 1877. 36 hours after birth pimples and bladders formed about the privates and navel, which parts swelled, turned black, and became hard; no sores. Infant died 21st August. The particular powder was used in this instance from birth. Three months afterwards some of the remainder of this powder was used for dusting sores on the head, face, and arms of a sister aged four years who had recently suffered from measles. These sores became much worse and inflamed; those on the arms have left large scars. ”

“ Accordingly, she sought out the most revolting cases of disease, and made appointments with the sufferers to meet her at her home, where kneeling before them while they sat, she washed and dressed their loathsome sores, contriving to stoop closely over their ulcerated limbs, so that nature might be crucified in every sense, and crushed in every feeling. ”

“ Seeing that we possess the means of rendering the action of the sores mild, which, when left to chance, are capable of producing violent effects; and seeing, too, that these sores bear a resemblance to the smallpox, especially the confluent, should it not encourage the hope that some topical application might be used with advantage to counteract the fatal tendency of that disease, when it appears in this terrific form? ”

“ When constipation exists one or two of Dr. Pierce's Pleasant Pellets taken daily, will fulfill the indication. The patient ought not to neglect to carry out all the [pg 450] hygienic recommendations heretofore given. The treatment of running sores is very simple. Cleanse them every day with Castile-soap and water, being careful not to rub or touch the surface of the sores. Use a clean sponge or a piece of clean muslin and saturating it with the warm water, hold it a few inches above the affected part, and squeeze out the fluid, allowing the cleansing stream to fall gently upon the open sore. ”

“ Finally, in order to prevent the child scratching the sores and the consequent danger of inoculation by the finger nails, it is a good practice to rub a small amount [608] of carbolated vaseline over the itching parts. It is frequently found necessary to have the little patient wear white woolen gloves to prevent scratching and infecting the sores. If a child scratches the sores on the face it will leave an unsightly mark which will stay for the rest of its life. ”

“ It often happens that women of a gross habit, at this period of life, have ulcerous sores break out about their ancles, or in other parts of the body. Such ulcers ought to be considered as critical, and should either be suffered to continue open, or have artificial drains substituted in their stead. Women who will have such sores dried up, are often soon after carried off by acute diseases, or fall into those of a chronic nature. ”

“ Bed-sores are most frequently met with in old and debilitated patients, or in those whose tissues are devitalised by acute or chronic diseases associated with stagnation of blood in the peripheral veins. Any interference with the nerve-supply of the skin, whether from injury or disease of the central nervous system or of the peripheral nerves, strongly predisposes to the formation of bed-sores. Prolonged and excessive pressure over a bony prominence, especially if the parts be moist with skin secretions, urine, or wound discharges, determines the formation of a sore. ”

“ It seldom kills and when properly treated it is easily cured. Of course it leaves one weak for a while, and not able to do much work. I wish, though, that I knew what to do to keep the mud from making sores on the men. I am more afraid of the sores than I am of the fever."
"I don't know anything about medicine," said Walter thoughtfully, "but it is evident that the sores come from germs or poisons in the mud. Now if the men would put carbolic acid in the water when they bathe morning and night and then put on some carbolic salve, I believe it would check or kill that which makes the sores." ”

“ Bed sores
, or pressure sores, are caused by continued pressure upon the skin. The weight of the [170] body, or of a part of the body, if it comes for a long time upon one place finally interferes with the circulation in the tissues on which the part rests, and consequently interferes with the nutrition of the affected part. Any tissue to which the blood is not bringing all its necessary food supply tends to lose its tone, to become weak, and if the condition persists, to break down altogether. ”

“ The direct cause of bed sores then is pressure, and pressure is aggravated by moisture, wrinkles in the bed clothes, crumbs or other hard particles, lack of cleanliness, friction of any kind, or by rough, careless handling. Bed sores occur most often over bony prominences, such as the end of the spine, elbows, heels, shoulders, hips, ankles, and knees, but they may form anywhere, even on the ears or back of the head. ”

“ Fresh gathered and chewed, the leaves are applied to running ulcers of the head, ulcerations of the mouth, gatherings, and condylomatous sores. A decoction of the leaves is very useful also for burns and sprains. Beaten up and applied with the juice of the strutheum,3031 they turn the hair red. The blossoms, applied to the head with vinegar, relieve head-ache, and the ashes of them, burnt in a pot of raw earth, are curative of corrosive sores and putrid ulcers, either employed by themselves, or in combination with honey. ”

“ They do not itch. (b) Moist lumps and warts form in the crutch, around the scrotum (purse) and the outlet of the bowel. (c) Ulcerated sore throat. Large deep ulcers form on each tonsil, having ragged undermined edges. (d) Iritis or inflammation of the eye may also occur. These symptoms, even if untreated, tend to heal, but always leave more or less marked traces behind.
The discharge from either primary or secondary sores is infectious and may convey the disease, so that great care needs to be taken in handling such sores. ”

“ The external examination of the body, before handling, is a matter not to be neglected. Should any syphilitic sores or foul ulcers be present, the utmost caution must be used in handling the body, as the pus, which is a most virulent poison, might find its way into the system through some abrasion of the skin. ”

“ They appear as pustules, which are rapidly converted into small, acutely inflamed ulcers with sharply cut, irregular margins, which bleed easily and yield an abundant yellow purulent discharge. They are devoid of the induration of syphilis, are painful, and nearly always multiple, reproducing themselves in successive crops by auto-inoculation. Soft sores are often complicated by phimosis and balanitis, and they frequently lead to infection of the glands in the groin. ”

“ It is, indeed, very rare to meet with sores in these situations that are not thus accounted for: certainly such as are by recurrence deep, extensive, and troublesome, are not seen unless in those who have suffered from mercurial medicines. ”

“ In cases of malignant sores or ulcers on any part of the body, the doctors treated them by applying dirt or earth, and in warm weather would excavate a place in the ground and put the patient in it, either in a sitting or recumbent position, as the nature of the case required, and cover the affected part with earth for several hours, daily. ”

“ Both large and small sores can be treated. The patient’s skin around the sore is protected by a thick layer of vaseline, and the surface of the ulcer powdered with potassium permanganate, which is kept in position by a pad of gauze and a bandage. ”

“ As an external local stimulant, or escharotic, the dried leaves in powder are applied to warts, flabby ulcers, and carious bones; and the expressed juice diluted, or an infusion of the leaves, as a lotion to gangrenous sores, scabies, and tinea capitis; or mixed with lard and wax as an issue ointment. The German writers speak very highly of its effects as a poultice to old and obstinate sores. Officinal Prep. Oleum Volatile Juniperi Sabinæ. E.D. (the dose of which is from one to three minims.) Extractum Sabinæ. D. (a very inert preparation.) ”

“ Yet many a syphilitic may and does think himself cured, and may marry in good faith, or be allowed a health certificate, only to become positive again. He may then develop new sores without his knowledge even, and perhaps infect his wife, or may himself in later years develop some of the serious consequences of the disease. ”

“ I am induced to think it does cease [Footnote: It is very easy to procure pus from old sores on the heels of horses. This I have often inserted into scratches made with a lancet, on the sound nipples of cows, and have seen no other effects from it than simple inflamation.] , and that it is the thin, darkish- looking fluid only, oozing from the newly-formed cracks in the heels, similar to what sometimes appears from erysipelatous blisters, which gives the disease. Nor am I certain that the nipples of the cows are at all times in a state to receive the infection. ”

“ All that he does to his sores is, to scrape them; they are not bound up with soft rags, nor mollified with ointment, not washed or kept clean: no healing plasters laid on them, no opiates, no anodynes, ministered to the poor patient, to alleviate the pain, and compose him to rest, nor any cordials to support his spirits; all the operation is the scraping of the ulcers, which, when they were come to a head, and began to die, made his body all over like a scurf, as is usual in the end of the small-pox. ”

“ The best method of managing such sores is, to take care that the patient’s constitution does not suffer by confinement, or improper medicine, and to apply nothing to them besides simple ointment spread upon soft lint, over which a poultice of bread and milk, with boiled camomile-flowers, or the like, may be put, to nourish the part, and keep it soft and warm. Nature, thus assisted, will generally in time operate a cure, by throwing off the diseased parts of the bone, after which the sore soon heals. ”

“ Attention must be paid in all treatment of sores or wounds to the proper cleansing and boiling of all materials and instruments used. Wash the hands in hot water and M'Clinton's soap, using a nail-brush, before touching or dressing a sore. ”

“ The enfeebled and disordered state of all the functions of the body evidently points out such a malignity of the fever as cannot be owing to the affection of the uvula or tonsils, which in other distempers we often see ulcerated and eaten away, without any danger of the patient’s life. These sores, therefore, like pestilential buboes, point out the nature of the disorder ”

“ Knots of superficial veins may often be seen beneath the skin.
As we have before remarked, these various species of ulcers are merely modifications of one form of chronic sore. The patient may assert that he enjoys excellent health, but if we question him closely, we find that the sore irritates him, and that there is sufficient constitutional disturbance to prevent the healing powers of nature from effecting a cure.
Treatment. The cure of these sores is necessarily slow, and who ever expects to obtain immediate relief will be disappointed. ”
