“ As the neck has much to do with the arm, we should keep a living picture of the forms of each bone, how and where it articulates with others, how it is joined by ligaments, what blood vessels, nerves and muscles cross or range with it lengthwise, because to overlook a small nerve and blood vessel you may fail to remove a goitre, and all diseases of the head, face and neck. ”

“ The lower extremities resemble the upper very closely in the arrangement of the bones, muscles, arteries, and nerves, though modifications occur, due to the difference in function of the lower limbs. There is one long bone in the upper part or thigh, the femur, and two in the lower part or leg, the tibia and fibula, while over the knee-joint is the patella or knee-cap. The ankle has seven bones and the foot nineteen like the hand. ”

“ But another physician putting the arm into the state of supination, gives orders to extend the arm thus, and bandages it in this position, reckoning it the one according to nature, judging thus from the skin, and also fancying the bones to be thus in their natural position, because the bone which protrudes at the wrist, where the little finger is, appears to be in a line with the bone from which people measure the bone of the fore-arm. ”

“ In man the strong forward inclination of the nuchal portion of the occipital bone is considered to have a relation to the upright position. I can not see why it should not be interpreted in the same way in the fossil skull under consideration. ”

“ Some of the bones, as the skull, protect delicate parts. The brain is so soft and delicate that it would be very unsafe without its solid bony covering. The spinal cord also needs the protection which it finds in the strong but flexible back-bone. The bones help to move our hands and arms, and assist us in walking.
13. The Joints.—The places where two or more bones are fastened together are called joints. Some joints we can move very freely, as those of the shoulder and the hip. Others have no motion at all, as those of the bones of the skull. ”

The frontal bone, as its name implies, forms the fore part of the head or forehead. It joins the parietal bones above and the temporal bones on either side. At the lower edge are the supra-orbital arches, each with a supra-orbital notch or foramen on its inner margin for the passage of the supra-orbital vessels and nerve, the nerve most affected in neuralgia. Just above the arches on either side are the superciliary ridges, behind which, between the two tables of the skull, lie the frontal sinuses. ”

“ Of those instruments which, falling upon the head, wound the parts about the bone, or the bone itself, that which falls from a very high place, and the least on a level with the person struck, and which is at the same time very hard, very blunt, and very heavy, and which is the least light, sharp, and soft, such an instrument would occasion a fracture and contusion of the bone. ”

The occipital protuberance occupies the extreme posterior end of the skull when the glabello-occipital line is made horizontal, and so far from any part of the occipital region extending beyond it, this region of the skull slopes obliquely upward and forward, so that the lambdoidal suture is situated well upon the upper surface of the cranium. ”

“ After the brain has been removed, the skin is stripped off a limited area of the skull, and the bone at the place of junction of the sagittal and lambdoid sutures above the occipital foramen is filed away, so that only a point, like a bony outgrowth, is left. ”

“ Having therefore first shaven the hairs about the forehead, we must permit the lower jaw to move, and avoiding the place where the temporal muscles are seen to act, we are to make three straight and parallel incisions on the forehead, each having the length of two fingers, and descending to the bone, and being at the distance of about three fingers’ breadth from one another. ”

953. Why is the spinal cord placed in the grooves formed by the back-bone?
Being a very vital part of the system, and from the delicacy of its structure liable to injuries, it is set in the back-bone for protection; and so great is its security that it is only by force of an unusual kind that it can be injured.

“ THE LEG AND ANKLE.
115. Bony points.—The tubercle of the tibia (for the attachment of the ligamentum patellæ) , the sharp front edge called the shin, and the broad flat subcutaneous surface of the bone can be felt all the way down. The inner edge can be felt too, but not so plainly. The lower third is the narrowest part of the bone and the most frequent seat of fracture. ”

“ The first division takes in head, neck, chest, abdomen and pelvis. The second division takes in head, neck, lower and upper arm and hand. The third division takes in foot, leg, thigh, pelvis and lumbar vertebra. I make this division for the purpose of holding the explorer to the limits of all supplies. In the ellipse of the chest is found all vital supplies; then from that center of [Pg 30] life we have two branches only, one of the arm, and one of the lower limb. In each division we have five points of exploration. ”

“ When, in the human subject, a process of bone passes up from the arch of the atlas, to meet the superior articular surface and convert the groove for the nerve and vertebral artery into a foramen, the process in question is a true oblique process. ”

“ To equalize this difference should therefore be the primary object of physical culture, for the harmonious structure of all its parts is an essential condition of a perfectly developed body. No malformation is more common in city recruits than a narrow chest. Besides spear-throwing, of which I shall speak further on, any exercise promoting the development of the shoulder muscles will tend to expand the chest, and thus remove the chief predisposing cause of consumption. ”

“ All this while the feet are resting on a board placed underneath them, the lower extremities being thus impeded in their movements, the head falls more and more forward, the muscles of the back are weakened and elongated, while those of the chest derive considerable strength from their constant exercise; the clavicles are unable to support the shoulders in their proper position, and therefore bend, or are forced from their natural situation; the cavity of the chest is diminished, and the founda­tion of numerous disorders is laid. ”

“ When a person is pained in the back part of the head, he is benefited by having the straight vein in the forehead opened.
69. Rigors commence in women, especially at the loins, and spread by the back to the head; and in men also, rather in the posterior than the anterior side of the body, as from the arms and thighs; the skin there is rare, as is obvious from the growth of hair on them. ”

“ Every region of the body exhibits fixed character proper to its surface. The neck, the axilla, the thorax, the abdomen, the groin, have each their special marks, by which we know them; and the eye, well versed in the characters proper to the healthy state of each, will soonest discover the nature of all effects of injury—such as dislocations, fractures, tumours of various kinds, &c. By our acquaintance with the perfect, we discover the imperfect; by a comparison with the geometrically true rectangled triangle, or circle, we estimate the error of these forms when they have become distorted ”

“ Joints.—Some bones are hinged upon each other. A bone hinge is a joint. The rings of the backbone are held together by very tough pads of flesh. Each pad lets the backbone bend only a little, but altogether they let us bend our backs in any direction. ”

“ Its occlusion may be effected by means of a sterilized wooden match or bone peg, by a small piece of bone derived from those osseous fragments which have been removed during the operation, or with [147] the aid of a piece of catgut; in each case the occluding medium should be well driven into the foramen. ”

“ On looking at the skull we find its base or occipital portion forming a right angle with the obliquely rising upper surface, and a bold transverse ridge is formed by the union of the occipital to the parietal bones, which latter advance above the frontal bones, and form the most elevated portion of the skull. ”

“ Recognizing in the scapular bones the ribs of the occipital centrum, the anatomists of this school of course consider them, when found articulated to the occiput, as in their proper and original place, and as in a state of natural dislocation when removed, as in all the reptiles, birds, and mammals, farther down. We find Professor Oken borrowing support to his hypothesis from this view. The limbs, he tells us, are simply ribs, that in the course of ages have been set free, and have become by development what they now are. ”

“ In all this delicate mechanism which pervades every individual side-arm, we see provision for the double purpose of attaching itself to extraneous bodies, and apprehending its prey. Five of these arms are set off from each of the largest joints of the vertebral column. At Pl. 53. Fig. 7. a. we see the bases, or first joints of these side-arms articulating with the larger vertebræ, and inclined alternately to the right and left, for the purpose of occupying their position most advantageously for motion, without interfering with each other, or with the flexure of the vertebral column. ”

“ The same combination is seen in an exaggerated degree in the bones of limbs that are paralysed; in the case of adults, atrophy of bone predominates; in children and adolescents, defective formation is the more prominent feature, and the affected bones are attenuated, smooth on the surface, and abnormally light. ”

“ We are told, and with perfect truth, that in man the occipital foramen--the aperture through which the brain is connected with the spinal cord--is so placed that the head is nearly in equilibrium when he stands upright. ”

“ The shoulder, in order to allow of extensive movement, has no osseus or ligamentous strength; it is, therefore, frequently dislocated. The wrist and ankle are rarely dislocated; as the result of violence at the wrist the radius gives way, at the ankle the fibula, these bones being relatively weaker than the respective joints. ”

“ How essential it is for the eye to be taught to recognise instantly a change of form or of colour. It is by comparing an injured limb with the sound one that the surgeon often learns by a look the nature of the injury or discovers by their tint the most appropriate management of the contents of a hernial sac. ”

“ I saw every degree of injury to the trunks, from notching to complete solution of continuity, and in some cases destruction and disappearance of pieces from one to two or more inches in length. Such lesions as the latter were most common in the forearm. In this segment of the limbs tracks of varying degrees of longitudinal obliquity are readily produced, [Pg 345] whether the patient be in the upright or prone position, since the upper extremities are commonly in forward action whichever position is assumed. ”

“ A Light Extension is to be made in the Fingers to reduce 'em, and a small Dressing is to be prepar'd for every Finger, almost like that of the Arm. The Fingers are to be somewhat bent, and the inside of the Hand is to be trimm'd with a Bolster, to retain 'em in this Situation. The Bolster is also to be stay'd with a Band, and the Arm to be put into a Scarf. ”

“ The supra-occipital bone is visible from behind; its top is covered and partly fused with a continuation of the parietals, which are, like the frontals, fused into an unpaired mass. The occipital condyle is formed entirely by the basi-occipital bone, so far as the articulating facet is concerned, but it is supported on either side by a lamella from the lateral occipitals. ”

“ No less obtuse than the back are the middle of the back of the neck, the middle of the forearm, and the middle of the thigh; and these parts have but rare experiences of irregular foreign bodies. The crown of the head is occasionally felt by the fingers, as also the back of one hand by the fingers of the other ”
